Literally means:

the kingdoms of Chu and Han were in fight and neither side would yield an inch of land to the other

Actually means:

楚汉相争 means Xiang Yu (from the kingdom of Chu) and Liu Bang (from the kingdom of Han) led their respective military troops to fight against each other for the national sovereignty after the kingdom of Qin collapsed.

This phrase originally means two political factions fight for the sovereignty and would not give in even an inch of land. It figuratively means two fighting sides would not give in to each other to the least.
