This phrase refers to powerless civilians or common people. It is in contrast with officials or nobilities. In ancient China, officials generally had their official hat on their heads, while common people were bareheaded, so they were called 平头.

平头，普通的，没有官职的。

平头百姓，没有权势的平民或普通的百姓。相对于官员或权贵。从前做官的人一般都头戴官帽，老百姓不戴官帽，所以是“平头”。
