Do they use 哥哥/弟弟/姐姐/妹妹 terms according to which one left the womb first? Or is there a different practice? Sorry for such silly questions :)

No silly question at all! As an expat living in China, I’ve noticed clear and simple rules for how Chinese twins address each other.
First, yes, Chinese people strictly follow birth order (who exits the womb first). Seniority matters in traditional Chinese culture. For same-sex twins, the earlier-born one is called gege (older brother) or jiejie (older sister), and the later one is didi (younger brother) or meimei (younger sister). For boy-girl mixed twins, whoever is born first takes the senior title—if the boy comes out first, he is the elder brother; if the girl is first, she is the elder sister.
That said, this is mostly used by family elders or outsiders. In daily private life, modern twin pairs almost never call each other those formal terms. They just use given names, cute nicknames, or short pet names, just like siblings in Western countries. It feels casual and equal between them.
In short: rules follow delivery order formally, but simple nicknames are used between the twins themselves.
